Volunteer Release & Waiver
Please stay home if you do not feel well or are displaying symptoms of COVID-19.

All volunteers must read and sign this form. Anyone under 18 must have a parent's signature. Please read before signing, as this constitutes the agreement as a volunteer and the understanding of your working relationship with SBP. Thank you!

I, the Volunteer, desire to work as a volunteer for SBP and engage in activities related to cleanup, community support, and/or construction work related to disaster relief. I hereby freely and voluntarily, without duress, execute this Waiver under the following terms:

  1. Waiver and Release.
  2. I, the Volunteer, on behalf of myself, my estate and my heirs, release and forever discharge and hold harmless SBP and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with SBP, and any travel or lodging associated therewith. I understand and acknowledge that this Waiver discharges SBP from any liability or claim, or claim for damages that I, the Volunteer, may have against SBP with respect to bodily injury, personal injury, illness, death, or property damage or theft that may result from my participation on the SBP work site, including any damages which may be caused by or arise from SBP’s negligence. I also understand that SBP does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage or theft.

  3. Medical Treatment Consent and Waiver.
  4. I, the Volunteer, hereby authorize SBP to provide to me with first aid and, through medical personnel of its choice, medical assistance, transportation, and emergency medical services (collectively, “Medical Treatment”) should an emergency arise. This consent shall in no way impose a duty upon SBP to provide such Medical Treatment. Furthermore, I understand that I am responsible for all costs associated with any such Medical Treatment and I waive and release any claims against SBP arising out of any Medical Treatment, including the lack or timing thereof, made in connection with my volunteer activities with SBP.

  5. Assumption of the Risk.
  6. I, the Volunteer, have chosen to travel to the work site, and understand that volunteering with SBP entails a risk of physical injury, and often involves hard physical labor, heavy lifting, and other strenuous activity; and that some activities may take place on ladders and building framing other than ground level. I certify that I am in good health and am physically able to perform this type of work, and understand that I am volunteering for SBP at my own risk. I hereby expressly and specifically assume the risk of injury or harm in my volunteer work with SBP release and SBP from all liability for injury, illness, death, or property damage resulting from the activities of my volunteer work with SBP.

  7. Risk Related to COVID-19.
  8. I, the Volunteer, understand that providing services in support of SBP's disaster victims may place me at greater risk of exposure to the COVID-19 virus. SBP is not able to determine the possible level of risk of exposure during this service. I understand that I must be prepared to self-quarantine should I begin showing symptoms of COVID-19 or come in contact with an individual diagnosed with the virus. It is my responsibility to notify my SBP supervisor if I begin to display symptoms.

I am aware that it is my obligation to follow appropriate local, national, and international guidelines for safe conduct to reduce contact with and spread of COVID-19. I will follow all health, hygiene, disinfection and safety directions issued by SBP team members.

  9. Media Release.
  10. I, the Volunteer, grant and convey unto SBP all right, title, and interest in any and all photographic images and video or audio/digital recordings made by SBP, its agents or employees, during my volunteer work for SBP, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

  11. Agreement Regarding Accommodations.
  12. In the event that SBP arranges accommodations, I, the Volunteer, understand that SBP is not responsible or liable for my personal effects and property and that SBP will not provide lock up or security for any items. I will hold SBP harmless in the event of theft or for loss resulting from any source or cause. I further understand that I am to abide by all rules and regulations that may be in effect for the accommodations at that time.

  13. Other.
  14. I expressly agree that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Louisiana. Any litigation arising out of this agreement shall be resolved by a court located in New Orleans, LA I agree that in the event that any clause or provision of this Waiver shall be held to be invalid, illegal, or incapable of being enforced by any court of competent jurisdiction, such clause or provision shall be excluded to the extent of such invalidity, illegality, or unenforceability; all other terms herein shall remain in full force and effect.
